Submit your vision claims online or by mail through My Canada Life at Work for timely reimbursement.

Vision Claims


Vision benefits under the Active Plan and the 60+ Plan help cover eligible vision care expenses. Coverage limits differ by plan, and claims are submitted through Canada Life.


How to Make a Vision Claim


1. Gather Required Documentation


Before submitting your claim, collect receipts that clearly show:

  • Date of service or purchase

  • Description of the service or product (for example, eye exam, glasses, or contact lenses)

  • Cost and proof of payment

If applicable, include a copy of your prescription for corrective lenses.


2. Submit the Claim


Option 1: Online Submission

  • Register or log in to My Canada Life at Work

  • Submit a healthcare expense claim

  • Upload scanned copies of your receipts and supporting documentation

Option 2: Mail Submission

  • Download the claim form from www.benefitsoffilm.com or request it from the Benefits Office

  • Complete the form with your personal and plan details

  • Attach receipts and any required supporting documents

  • Mail the completed form and documentation to Canada Life at the address listed on the form


Laser Eye Surgery


Laser eye surgery may be eligible for coverage under the plan.

Before proceeding:

  • Contact Canada Life to confirm eligibility and coverage limits

  • Submit a written estimate from the surgical provider if requested

  • Wait for confirmation before scheduling the procedure


Payment of Claims


  • Approved vision claims are processed according to plan coverage limits

  • Payments are issued by direct deposit for online submissions or by cheque for mailed claims

  • Ensure your banking information is up to date in My Canada Life at Work
