Dental Claims - Active & 60+ Plans

For timely dental claim reimbursement, follow the submission process through Canada Life, either online or by mail.

Dental benefits under the Active and 60+ Plans help cover a range of dental services, including preventive care and restorative treatment, subject to plan limits, percentages, and eligibility rules.


How to Make a Dental Claim


Using the Pay Direct Option

Many dental offices may be able to submit claims directly to Canada Life on your behalf.

  • Present your Canada Life benefits card at your appointment

  • The plan pays the covered portion directly to the dental provider

  • You pay any uncovered amount at the time of service


Submitting a Dental Claim Yourself


If your dental provider does not submit claims directly, you can submit the claim yourself.


1. Gather Required Documentation

Obtain an itemised receipt from your dental provider showing:

  • Date of service

  • Description of the procedure

  • Total cost and proof of payment

  • Dentist’s licence number and contact information


2. Complete the Dental Claim Form

Complete all required sections, including your plan and member details.


3. Submit the Claim

Online

  • Log in to My Canada Life at Work

  • Upload scanned copies of the claim form and receipts

By Mail

  • Mail the completed claim form and receipts to Canada Life at the address listed on the form


Pre-Treatment for Major Dental Procedures


For major procedures such as crowns, bridges, or dentures, you are encouraged to submit a treatment plan to Canada Life before starting treatment.

  • Ask your dentist to prepare a treatment plan

  • Submit it to Canada Life for review and cost confirmation

  • Approved treatment plans are valid for six months


Payment of Benefits


  • With Pay Direct, Canada Life pays the dental provider directly

  • For self-submitted claims, payments are issued by direct deposit (online submissions) or cheque (mail submissions)


Coordinating Dental Benefits


If you or your spouse has dental coverage under another plan:

  • Submit claims first to the primary plan

  • Submit any remaining balance to the secondary plan

For dependent children, claims must be submitted first to the plan of the parent whose birthday falls earlier in the calendar year.

  • Confirm coverage before starting treatment

  • Keep copies of all receipts and claim submissions

  • Submit claims as soon as possible to avoid delays
