A: Yes, the lifetime maximum coverage for gender-affirming procedures is $25,000 per member. This includes both top and bottom surgeries as well as other gender-affirming procedures.

Max had spent years exploring his gender identity, and after much consideration, he decided it was time to begin his gender-affirming surgeries. He had already completed hormone replacement therapy (HRT) and felt a sense of peace, but now he was ready to undergo top surgery and other procedures to align his physical appearance with his true self.
