Making a Claim after Accidental Death
Q: What should Tom's family do if he dies due to an accident?
A: If Tom had Active Plan coverage at the time of the accident, AGA Benefit Solutions will provide his beneficiary with Accidental Death & Dismemberment (AD&D) claim forms. A lump-sum payment will be made to his named beneficiary—or to his estate if no beneficiary is on file.
Tom never liked to dwell on worst-case scenarios, but he understood how important it was to be prepared. In the event of an accident, he had taken steps to ensure his family would be supported.
If the unthinkable happened and Tom died due to an accident while covered under the Active Plan, his wife, Emma, would receive Accidental Death and Dismemberment (AD&D) claim forms from AGA Benefit Solutions. She would complete and submit the forms so the benefit could be processed.
If Tom had named Emma as his beneficiary, the lump-sum payment would go directly to her. If no beneficiary was on file, the benefit would be paid to his estate.
Knowing that this process was in place gave Tom a quiet sense of peace. Even in tragedy, he could rest assured that his family would have some financial support to help them through an incredibly difficult time.
