Paramedical services under the Active Plan and the 60+ Plan provide coverage for eligible services performed by licensed practitioners, subject to plan rules, coverage limits, and practitioner eligibility.
How to Make a Paramedical Services Claim
1. Gather Required Documentation
Obtain receipts from the licensed practitioner that clearly show:
Practitioner’s name, credentials, and registration number
Description of the service provided
Date of service
Total cost and proof of payment
2. Complete Your Claim
Paramedical claims are submitted to Canada Life as healthcare expense claims.
Download the claim form from www.benefitsoffilm.com, or
Submit your claim online through My Canada Life at Work
Ensure all required member and service details are completed accurately.
3. Submit the Claim
Option 1: Online Submission
Log in to My Canada Life at Work
Submit the healthcare expense claim and upload scanned copies of your receipts
Option 2: Mail Submission
Attach receipts to the completed Canada Life claim form
Mail the form and receipts to Canada Life at the address listed on the form
Payment of Benefits
Coverage and reimbursement levels vary by service and by plan
Approved claims are paid by direct deposit for online submissions
Mailed claims are paid by cheque

If you have coverage under another plan, submit claims first to the primary plan, then submit any remaining eligible balance to the secondary plan
