Life Insurance Claims

Life Insurance benefits under the Active Plan and the 60+ Plan provide financial support to beneficiaries in the event of a member’s death.

• Under the Active Plan, coverage includes Basic Life Insurance and any Optional Life Insurance elected by the member.

• Under the 60+ Plan, members are eligible for a $5,000 death benefit, and any Optional Life Insurance elected while covered under the Active Plan may continue, subject to plan rules.

  
  

How to Make a Life Insurance Claim

1. Notify AGA Benefit Solutions

As soon as possible following a member’s death, an executor, family member, or friend may begin the claims process by contacting AGA Benefit Solutions.

AGA Benefit Solutions will explain the next steps and provide the required claim forms.

2. Complete the Claim Forms

The beneficiary will be asked to complete and sign the Life Insurance Claim Form provided by AGA Benefit Solutions.

3. Provide Supporting Documentation

Supporting documentation will be required to process the claim. This may include:

• A certified death certificate

• Confirmation of beneficiary information

• Additional documentation, if requested, depending on the circumstances

AGA Benefit Solutions will advise what documentation is required.

4. Submit the Claim

Submit the completed claim form and required documentation to AGA Benefit Solutions by mail or email, as instructed.

Payment of Benefits

Once the claim is approved:

• Life insurance benefits are paid as a lump sum

• Payment is made to the named beneficiary, or to the estate if no beneficiary is on file

  
  

Important Notes

• Keeping beneficiary information up to date helps avoid delays

• Optional Life Insurance under the 60+ Plan is only available if it was elected while covered under the Active Plan
