To ensure timely processing of your Critical Illness claim, please follow the steps below.
Critical Illness Coverage
Critical Illness coverage under the Active Plan provides financial support in the form of a lump-sum payment to members diagnosed with a covered life-threatening condition. This benefit is intended to help ease financial pressure during treatment, recovery, or time away from work.
How to Make a Critical Illness Claim
1. Notify Allstate Insurance
Notify Allstate, the insurer for Critical Illness coverage, as soon as possible after you are diagnosed with a covered condition.
Claim forms are available at www.benefitsoffilm.com.
If needed, you may also contact the Benefits Office for general guidance on starting the claims process.
2. Complete the Claim Form
The Critical Illness claim form must be completed in full. The form may include information provided by both the member and their attending physician, along with authorisation to allow the insurer to review relevant medical records.
3. Provide Supporting Medical Information
You will be required to submit medical evidence confirming your diagnosis. This may include:
Medical reports or test results
A completed physician’s statement
Additional documentation requested by the insurer
4. Submit the Claim
Submit the completed claim form and supporting documentation directly to Allstate using the submission method indicated on the claim form.
Ensure all sections are completed accurately to avoid delays.
Submission Deadlines
Initial notice of claim: within 30 days of diagnosis
Submission of medical proof: within 90 days of diagnosis
If you are unable to meet these deadlines, submit the claim as soon as reasonably possible, along with an explanation. The insurer will not accept a claim notice beyond one year from the diagnosis date.
Claim Review and Payment
Allstate will review the submitted claim and supporting medical documentation. Additional information may be requested if required.
Once approved, the lump-sum benefit is paid directly to the member. The funds may be used at the member’s discretion.
