For accidents resulting in death or serious injury, follow the steps below to ensure your claim is submitted and processed as smoothly as possible.
Accidental Death & Dismemberment (AD&D) Coverage
The Accidental Death & Dismemberment (AD&D) benefit under the Active Plan provides financial protection to members and their beneficiaries in the event of accidental death or certain serious injuries resulting from an accident. Coverage applies worldwide, 24 hours a day.
How to Make an Accidental Insurance Claim
1. Notify AGA Benefit Solutions
Contact AGA Benefit Solutions as soon as possible following the accident or death.

You may be asked to provide details such as the date, location, and nature of the accident or injury.
2. Obtain the Required Claim Form
AGA Benefit Solutions will provide the appropriate AD&D Claim Form and guide the claimant or beneficiary through the next steps.
3. Provide Supporting Documentation
Depending on the nature of the claim, supporting documentation may be required.
For accidental death claims, this may include:
A certified death certificate
An accident or coroner’s report confirming the cause of death
Proof of beneficiary designation
For injury or dismemberment claims, this may include:
Medical documentation from a licensed healthcare provider detailing the injury
An accident report (for example, police or workplace documentation)
4. Complete and Submit the Claim
The claimant or beneficiary must:
Complete the claim form in full
Attach any requested supporting documentation
Submit the completed claim using one of the following methods:

Submission Deadline
Claims must be submitted within 15 months of the date of the accident or death.
If it is not reasonably possible to submit the claim within this timeframe, it must be submitted as soon as reasonably possible thereafter.
Claim Review and Payment
AGA Benefit Solutions will review the claim and supporting documentation. If additional information is required, they will contact the claimant or beneficiary directly.
Once approved:
Accidental death benefits are paid to the named beneficiary or, if no beneficiary is designated, to the estate
Injury or dismemberment benefits are paid directly to the member
Important Notes
AD&D benefits are not available under the 60+ Plan
AD&D coverage ends when a member enrols in the 60+ Plan
