For timely vision claim reimbursement, follow the submission process through Canada Life, either online or by mail.


How to Make a Claim


1. Gather Required Documentation

Before submitting a claim, ensure you have:

  • An itemized receipt from the optometrist, ophthalmologist, or retailer detailing:

    • The date of service.

    • The type of service or product (e.g., eye exam, glasses, contact lenses).

    • The cost and proof of payment.

  • A copy of your prescription for corrective lenses, if applicable.


2. Submit the Claim

Option 1: Online Submission

  • Register or log in to the My Canada Life at Work portal.

  • Navigate to the claims section and select "Vision Claims."

  • Upload the receipt and supporting documentation.

  • Complete the online form, providing details of the expense.


Option 2: Paper Submission

  • Obtain a Vision Claim Form from the benefits office or download it from www.benefitsoffilm.com.

  • Fill out the form with your personal information, plan details, and specifics about the expense.

  • Attach the original receipt and a copy of your prescription.

  • Mail the completed form and documentation to Canada Life at the address provided on the form.


3. Pre-Approval for Laser Eye Surgery

  • Contact Canada Life to confirm eligibility and coverage limits for laser eye surgery.

  • Submit a pre-authorization request along with a written estimate from the surgical provider.

  • Await confirmation before proceeding with the procedure.


Deadlines


  • Submit all vision claims within 12 months of the service or purchase date.

  • Late submissions may not be reimbursed unless valid reasons for the delay are provided.


Reimbursement


  • Reimbursement is processed based on the plan’s coverage limits.

  • Payments are typically deposited directly into your account for online submissions or mailed by cheque for paper submissions.

  • Ensure your bank details are updated in the My Canada Life at Work portal to avoid delays.

  • For out-of-network providers or services, retain all documentation to ensure claims are processed smoothly.
