Travel claims under the Active Benefits Plan provide coverage for emergency medical assistance and out-of-country care for members and their eligible dependents. This benefit ensures that medical emergencies while traveling outside your province or country are covered, alleviating financial and logistical stress.
How to Make a Claim
1. Emergency Assistance While Traveling
Contact the Travel Assistance Provider Immediately:
Call the emergency number listed on your benefits card or in the booklet.
Provide your name, member number, and details of the emergency.
The travel assistance provider will guide you on the steps to take and may pre-authorize treatments.
2. Gather Documentation
Obtain and retain all documents related to the emergency, including:
Medical bills, receipts, and invoices for services received.
Doctor’s or hospital’s detailed reports of the diagnosis, treatment provided, and necessity of care.
Prescription records for medications obtained during travel.
3. Complete the Claim Form
Download the Out-of-Country Claim Form from www.benefitsoffilm.com or obtain it from the benefits office.
Fill in all required sections, including personal information, travel details, and the nature of the emergency.
4. Submit the Claim
Online Submission:
Log into My Canada Life at Work and upload the completed claim form along with all supporting documents.
Mail Submission:
Send the completed claim form, original receipts, and supporting documents to Canada Life at the address listed on the form.
Deadlines
Claims must be submitted within 12 months of the date the medical expenses were incurred.
Late submissions may require an explanation for the delay and are subject to approval.
Reimbursement and Payments
Claims are reviewed by Canada Life to determine eligibility and coverage limits.
Payments are reimbursed directly to your bank account if your claim is approved. For manual submissions, cheques may be issued.
