For timely processing of your rehabilitation claims, follow the guidelines below to ensure reimbursement for eligible services.


How to Make a Rehabilitation Claim


1. Residential Rehabilitation


Obtain and retain all receipts from the rehabilitation centre, detailing:

  • Dates of admission and discharge.

  • Total cost of the programme.

  • The centre’s licensing and contact details.


Provide a certificate of completion or confirmation of attendance from the facility.


Note: The centre must specialise in drug and/or alcohol misuse, and a licensed doctor must be on staff 24/7.


2. Non-Residential Rehabilitation


Obtain receipts for all outpatient services, including:

  • Counselling or therapy sessions.

  • Medication prescribed as part of the treatment plan.


Include a letter of recommendation or referral from:

  • A licensed doctor

  • An EFAP provider

  • A psychologist or certified addiction counsellor


Note: The provider must specialise in drug and/or alcohol misuse, and a licensed doctor must be available on staff 24/7.


3. Submit the Claim


Rehabilitation claims must go through the Union office.


  • Request the claim form from the Union office.

  • Complete the form and attach all required documents.

  • Return everything to the Union office—they’ll send it to AGA, who submits it to Canada Life.


Deadlines


  • Submit claims within 12 months of completing the rehabilitation program or outpatient service.

  • Late claims may require a valid explanation and are subject to approval.


Reimbursement


  1. Reimbursement is based on the plan’s coverage limits for rehabilitation services.

  2. Payments are made directly to your bank account (for online submissions) or via cheque (for mailed claims).

  • Contact the Benefits Office or EFAP at 1-800-667-0993 for guidance and assistance with finding licensed rehabilitation providers.

  • Confirm your coverage details, including annual or lifetime limits, before starting a program.
