In the event of a member’s passing, here’s how to file a claim for both basic and optional life insurance coverage, ensuring a smooth process during a difficult time.

Life Insurance Claims


The Life Insurance benefit under the Active and 60+ plans provides financial support to beneficiaries in the event of a member’s passing. This includes Basic Life Insurance and any optional coverage chosen by the member.


How to Make a Claim for Life Insurance


1. Notify AGA Benefit Solutions

  • Contact AGA Benefit Solutions immediately after the member’s passing:

  • Provide the member's full name, plan details, and date of death.


2. Obtain the Necessary Claim Forms

  • AGA Benefit Solutions will provide the Life Insurance Claim Form and guidance on additional documentation required.


3. Prepare Supporting Documents

  • Collect the following documents to support the claim:

    • Death Certificate: A certified copy issued by the appropriate authority.

    • Proof of Beneficiary Designation: Ensure the beneficiary’s name matches the records on file.

    • Additional Documentation: May include medical reports or a coroner’s report if requested.


4. Complete and Submit the Claim Form

  • The beneficiary must fill out and sign the Life Insurance Claim Form.

  • Submit the completed form along with all supporting documents to AGA Benefit Solutions via mail or email.


For Claims Under the 60+ Plan


  • Coverage under the 60+ Plan includes a reduced life insurance amount compared to the Active Plan.

  • The claims process remains the same:

    • Notify AGA Benefit Solutions.

    • Complete the required claim forms.

    • Provide all supporting documents.


Key Deadlines


  • Submit the claim as soon as possible after the member’s passing.

  • Claims should ideally be filed within 12 months to ensure timely processing.


Processing and Payment


  1. AGA Benefit Solutions reviews the submitted claim and documentation.

  2. If approved, the lump sum is paid to the named beneficiary or the estate (if no beneficiary is designated).

  3. Payments are usually processed within 30 days of claim approval.


Important Notes


  • Ensure beneficiary information is up to date to avoid delays in claims processing.

  • In cases where no beneficiary is named, the benefit will be paid to the member’s estate.
