How to Make a Critical Illness Claim
Notify Allstate Insurance
Contact Allstate Insurance (or the designated insurer for Critical Illness claims) as soon as you are diagnosed with a covered illness.

Obtain the Critical Illness Claim Form
Download the form from www.benefitsoffilm.com or request it from the Benefits Office.
The claim form typically consists of three sections:
Member Statement: Details about the diagnosis, condition, and personal information.
Physician Statement: Medical information provided by your attending physician.
Authorization: Signed consent allowing the insurer to access medical records.
Gather Supporting Documentation
Ensure you have the following:
A completed Physician Statement from your attending physician, including:
Diagnosis of the illness.
Date of diagnosis and details of the severity of the condition.
Medical reports, test results, and any other supporting documentation.
Additional documents as requested by the insurer (e.g., pathology or lab reports).
Submit the Claim
Submission Options:
Online: If the insurer allows, upload the completed forms and documents through their portal.
Mail: Send the completed claim form and supporting documentation to Allstate Insurance at the address specified on the form.

Deadlines
Initial Notification:
Notify the insurer within 30 days of the diagnosis.

Late Submissions:

Claim Review Process

Reimbursement
Once approved, the lump-sum payment will be issued directly to the member.
